The switch end OSFP has a finned top for additional cooling and the QSFP112 adapter end is flat topped and uses riding heat sinks on the OSFP cage. The ACC contains eight high-speed electrical copper pairs, each operating at data rates of up to 100Gb/s using 100G-PAM4 modulation. No, it won't affect. After port configuration and splitting, internally in the device, it is recognized as two separate logical ports. Each branch of the cable corresponds to a logical port, and they do not affect each other.

OSFP is very large and heavy, and the fiber can break easily. A two-branch cable will have 3 large transceiver ends, and a four-branch cable will have 5 transceivers. This will almost certainly easily lead to fiber breakage during installation - especially for a 30m long AOC.
